feature: callouts (?)

I have that idea of placing callouts directly in the code for quite a while.

The annotations from codly take away quite a piece of space at the right end of the code example.

I have seen a solution that is pretty close to what I have in mind that the guys implementing CodeHike have:

Hey,
I really like this idea, but I think I'll go the same way as zebraw instead and it will not be based on comments (I don't want to parse comments, it's too tricky, error prone, and brittle). I'll try and add this for 1.4 since I'd like to get 1.3 out soon-ish to fix warnings
