Fix tool-failure payload regression on main; ignore agent worktrees - #38

After LOU-U2 and LOU-U4 were merged together, a failed tool's message was
always flattened to {error}, dropping LOU-U4's per-issue validation detail
(toolArgsValidation.test.ts failed on main). A failure that already carries
a structured result now keeps it.
Also excludes .claude/ (agent worktrees are full checkouts) from vitest,
coverage, fallow and git.
