feat(observe): evaluator rules API - #751

Add the evaluator_rules table (migration 5) and CRUD under
/api/v1/evaluators: code rules ({ source }) and judge rules
({ model, rubric }) with a trace filter and sample rate. Rules are
stored only; nothing compiles source or calls a model yet.

z.enum's errorMap option is zod-3 only; the { message } form typechecks
and names the allowed kinds under both zod 3 and zod 4.

Online evaluator rules are stored in the observe daemon (evaluator_rules table + CRUD under /api/v1/evaluators), the base the code and judge evaluator runners build on.
