feat(chains): add Moralis and Subsquid Subgraph icons - #56

Both spec ids (MORALIS, SQDSUBGRAPH) already resolve to /chains/<id>.svg by the default convention; these were the only catalog entries without an icon file. Adds the two SVGs and a resolver test asserting they map to their files.
These four had light/white marks on no or near-white backing, so they were nearly invisible on white surfaces. Replace with the brand mark on a solid brand-coloured disc: Axelar/Stellar white-on-black, Casper white-on-red, Litecoin white Ł on #345D9D.
Cream e-planet mark on transparent → near-invisible on white. Place it on the Mars-rust brand disc (#ED4E33), matching the other circular icons.
Replace the inconsistent chain icons with uniform brand-colored circular discs. 46 marks sourced from web3icons.io, the rest from official brand assets (sqd.dev, union.build, kakarot.org, elys, side, etc.) or recreated to match official logos. Fixes washed-out icons (Axelar/Casper/Stellar/ Evmos/Litecoin) and standardises the set onto same-size brand discs that read on light surfaces.
Add avalanche-c.svg / avalanche-p.svg (Avalanche disc with a C/P corner badge) and point the AVALANCHEC/AVALANCHEP aliases at them, so the three Avalanche specs no longer render the same icon.
