docs: steward cycle 21, reconcile status with main through f51ef7e (#290) - #460

) Status: fold the three review-found repairs shipped in v0.4.0 into short entries; name the merge for every In flight entry that still said delivery was pending (#446, #448, #453, #441); drop entries whose work shipped in v0.4.0 and is recorded in its review (#418, #430, #435) or shorten them (#429, #404 candidate, CF-4 closeout); retire the v0.3.0 post-publication gates, which v0.4.0 supersedes; record the KWallet5 baseline pin (#456) in Next; move the iPhone secure-answer focus item to resolved rough edges (#455). Plans: add a dated state line to the six ledgers that had none (CF-7 context map, CR-4 overview, CR-5 integration and remote diffs, CR-6 isolation, CR-7 intents). No section that #452 (release-041) changes is touched. All relative links and anchors resolve. Coven-Harness: Claude Code / claude-fable-5-1
