stop hook: break the continuation loop without the host flag (mr-j30) - #217
Conversation
The Stop hook trusted the host's stop_hook_active to yield on the second pass, but Codex reports it false during hook-driven continuations (captured: dozens of block reasons, four yields), so the gate could refuse forever — a hang. Add a host-independent guard: a per-session state file counts consecutive refusals, both per blocker set (a small limit) and session-wide (a larger cap, for a loop whose blocker set keeps changing), and the hook stands down loudly at either limit, exactly as the host-driven second pass does. A pass, or a stand-down, clears the record, so a later session never inherits a spent count. The guard is best-effort (a state failure only skips it), so it can only fail toward blocking. It covers the missing-binary path too — the other refusal a session cannot clear from inside — and the limits are validated env overrides.

- Around line 217-220: Update the loop-break guarantee in the documentation to
clarify that refusal limits apply only when refusal state can be persisted; when
no record is readable and no state location is writable, repeated refusals may
continue without reaching either limit.
- Line 218: Update the STOP_KEY description in the refusal-count documentation
to clarify that when session_id is absent, the reported checkout is used as the
fallback key, so sessions in the same checkout share a refusal record and one
session’s pass or stand-down can clear another’s count.
Review comments at @hooks/pre-finish.sh:
- Line 138: Update the stop-gate state-directory selection around `mkdir -p
"$dir"` to reject directories not owned by the current user or writable by group
or others, checking the directory itself after creation. Preserve the existing
writable-directory check and fallback flow, and do not use a shared temporary
directory unless it passes these ownership and permission checks.
- Line 160: In block_is_repeat, serialize the full read-increment-write sequence
for each state file with a per-state-file lock, then write through a temporary
file in the same directory and replace the state file with mv. Do not rely on
atomic replacement alone; concurrent hooks must not lose counter increments.

Fixes bead mr-j30 (P2): the Stop gate could livelock — refuse forever — under Codex.
The bug
hooks/pre-finish.shtrusted the host'sstop_hook_activefield to yield on the second pass, but Codex reports it false during hook-driven continuations (captured 2026-09-26: dozens of block reasons, only four yields). A session blocked on something it cannot clear (metareview statusexits 1) would then be refused on every Stop, forever — a hang, which teaches operators to remove the hook. The fix
A host-independent loop guard in the Stop hook: a per-session state file counts consecutive refusals and, once the count reaches its limit, the hook stands down loudly on stderr — exactly as the host-driven second pass does — and clears the record.
must_cleartargets/verdicts), and a larger session-wide cap for a set that keeps changing. An empty/unreadable signature is its own signature, so it still counts.CODE==1blocker path, the broken-gate path (a persistently failingstatus), and the missing-binary path. A pass, or a stand-down, clears the record, so a later session never inherits a spent count.session_id, else the stable reported checkout — never a process id, which would change every turn and leave the guard inert exactly where it is needed.$XDG_CACHE_HOME→$HOME/.cache), falling back to the repository's git dir and only then the temp dir; it is created lazily (a session that never blocks leaves nothing behind). The whole mechanism is best-effort and fails toward blocking.Review
Reviewed with adversarial
review-looprounds (pi subagents), which caught: an empty signature short-circuiting before counting (so the cap never fired), a state location poisonable via a shared temp dir, the broken-gate path being unguarded, and — importantly — that making the key stable would trip the shell suite'stests/go/test-stop-hook.shcase 11 (its four no-session refusals are independent first passes); that suite now sets the guard aside and isolatesXDG_CACHE_HOME.metareview review pr-ready→ PASS;go test ./...,go vet,golangci-lint,shellcheckand the shell suite all clean.Residual advisory (confirmed, deferred): in the narrow case where the cache dir, the repo git dir and the temp dir are all unwritable the guard is inert (fails toward blocking); and a no-session host whose reported cwd moves between turns resets the count.
Note
Medium Risk
Changes Stop-gate enforcement semantics (stand-down after repeated blocks can bypass the gate) and adds best-effort persisted state with narrow fail-open edge cases when no safe state directory exists.
Overview
Fixes mr-j30: under Codex,
stop_hook_activestays false during hook-driven continuations, so the Stop hook could block forever on blockers the session cannot clear.hooks/pre-finish.shadds a host-independent loop guard that persists per-session refusal counts (private cache → repo git dir → temp, with ownership/mode checks viastop_guard_writable). It stands down loudly after configurable limits—the same blocker set (METAREVIEW_STOP_GUARD_LIMIT, default 3) or any repeated refusals in the session (METAREVIEW_STOP_GUARD_TOTAL, default 6)—not only whenstop_hook_activeis set. That applies to unresolved blockers (hashedmust_clearsignature), missingmetareview, and persistently brokenstatusexits; passes and stand-downs callstop_guard_clearso later runs are not stuck with a spent counter.Tests and docs:
internal/githooktest/stophook_layout_test.goadds coverage for continuation loops, session-wide cap, broken gate, pass/stand-down reset, and poisoned state dirs;tests/go/test-stop-hook.shisolates cache state and raises guard limits so existing “first pass must block” cases stay independent.
