feat!: remove workspaces.create (server retired POST /v1/workspaces) - #124

Every session owns its workspace. Drop CreateWorkspaceRequest and workspace_id on CreateSessionRequest (server now rejects it with 400). Examples and docs read the workspace from Session.workspace_id.
