My mistake from the earlier stack: a git add -A in #65 picked up two files from my local graft code-graph cache (graft/.cache/stats.json and a session JSON), and #66 and #68 updated them again. They are regenerable local state and don't belong in the repo.

This PR removes them from the index (git rm --cached, so local copies stay) and adds /graft/ to .gitignore. These are the same lines graft already wrote into the uncommitted local .gitignore.

graft/.cache/ files were accidentally committed with #65 (and later
updated by #66 and #68). They are a regenerable local cache of the graft
code-graph tool. Untrack them and ignore /graft/.
