[scanner] repeat-failure: dashboard row note, check states, no-data safeguard, coverage table and docs #14558
Description
Part of #14550 — operator-facing surface. Depends on #14557 (detector) and #14554 (neutral route). Target: v6 (open the PR with --base v6).
Scope (requirements C4, C5, C11, C12, C13, C15):
- New field on the agent status the dashboard renders (not a watchdog condition, not
BusyCondition): note "Repeating failure:<command>failed N times in M min (exit S)" / "(failed)", most recent + "and K others"; check state: on / "repeat-failure check not available for this backend" / "repeat-failure check not receiving data". Nothing shown when the switch is off. - C12 safeguard: covered agent with a turn in progress showing shell-tool activity and no record for
no_data_after_s(900) → not receiving data; file reads/edits alone and a still-running command don't count. - Code coverage table listing every
config.CLIBackendsandconfig.InferenceBackendsentry as covered (route, evidence incl.--version, shell-activity signal or "safeguard unavailable") or not covered (reason, follow-up issue); CI test fails if a backend is missing or docs table differs (C13). Only mark covered with committed real-run harness evidence (C4); otherwise not covered. - Docs: repeated-failure table in
src/docs/backend-support-tiers.md(advisor-lane style), dashboard docs for note and states.
Files (likely): src/pkg/agent status field + coverage table, src/pkg/dashboard row rendering, docs. Run bash src/scripts/check-api-reference-citations.sh --fix if server.go is touched.
Acceptance: C-AC9, C-AC10, C-AC11, C-AC12 (no check state when off). No time.Sleep( in tests.

Scanner triage (not starting work): this item depends on #14557 (detector + status field) and #14554 (neutral route), and both are still open with no PR as of this kick. The dashboard note, check states and the C12 no-data safeguard all render data those two produce, and the coverage table needs the routes they define, so implementing this first would only ship stubs. Deferring until #14557 and #14554 merge; the next kick after that picks this up.
