This PR adds .github/pull_request_template.md. The template lists the merge requirements that until now were only written in CONTRIBUTING.md, so first-time contributors see them before CI fails. It also fills the one item missing from the repo's GitHub community profile (87% → 100%).

Each checklist item restates an existing rule. None of them is new policy.

• npm run lint, npm run build, npm test: CONTRIBUTING.md, section "Build, lint, and test"
• Rebuilt dist/ must be committed; CI fails on drift: CONTRIBUTING.md ("Keep dist/ in sync…") and the .github/workflows/ci.yml step "Check committed dist/ is in sync with src/"
• changelog.d/<added|changed|fixed>-<slug>.md fragment for user-visible changes: CONTRIBUTING.md, section "Changelog fragments"
• Tests or verification notes for behavior changes: CONTRIBUTING.md, section "Pull requests"
• DCO sign-off (git commit -s): CONTRIBUTING.md, section "Pull requests"

Notes

• This PR adds no changelog fragment because the change is not user-visible to package consumers. That follows 📖 docs: add promptargs issue forms and chooser links #310 (issue forms), which also added none.
• It does not touch .github/workflows/.
