fix: treat a worker whose turn ended unanswered as still running - #38

Live on lich 0.58.0 a worker left a sleep running in the background and ended its turn; lich reported the errand unanswered at the open and the mod answered the Agent call as a finished subagent that never reported, while the worker kept going and its report still came later as a [lich] note. unanswered now maps to async_launched like pending. The unanswered test changes because the contract it pinned changed.
