Replaces the header raw API/curl modal with contextual in-place AI notes extraction inside AgendaDetail and FocusMode, moves personal access token management to AccountView settings, unescapes Milkdown bracket escaping in action outcome tags, fixes text wrapping/clipping across workspace headers, outcome badges, FocusMode topbar, and Actions/Decisions tables, and refreshes docs/assets/demo_meetflow.gif.
