fix(engine): drop a trace-question result a newer call superseded - #683
A trace question can be asked twice at once on a run: new spans change its evidence while the first model call is out, and the next flush asks again. Whichever call finished last wrote last, so a slow older call could raise a finding on evidence the newer call had answered no, or leave 'could not run' after it.
Drop a result once the run's evidence key for the question has moved on. A cleared key (the newer call found no model) keeps the answer.

Summary by CodeRabbit
Bug Fixes
Trace-question results now stay aligned with the latest available evidence when evaluations overlap. Older evaluations no longer replace newer decisions or add outdated failure findings, including when a newer evaluation has no decision model available.
Manual rechecks can supersede evaluations that are still in progress, so their later results do not override the recheck.
Tests
Added coverage for overlapping evaluations, changing evidence, failed evaluations, unavailable decision models, and manual rechecks.

Fixes#682
The trace-question path now tracks overlapping calls and prevents superseded results from updating findings or decisions. Regression tests cover older calls that finish before or after newer calls, including errors, rechecks, and unavailable models.

Track calls and reject superseded results packages/neatroots/src/engine/engine.ts, packages/neatroots/test/trace-question-order.test.ts
askTrace tracks per-question call order and passes a current-result predicate to decideAndApply. The method checks the predicate before recording failures or decisions and after second-look verification. Tests cover overlapping calls, rechecks, and unavailable models.
Feature: explain live check outcomes on each run #522: The issue concerns stale asynchronous live-check outcomes. This change addresses overlapping trace-question results, but does not add outcome explanations or persistence.
Possibly related PRs
neatlogs/neatroots#672: Both changes modify Engine.decideAndApply and gate writes associated with trace-question results.
Merge Risk: Moderate · up to f86e8
A trace can lose its only available decision and finding when overlapping calls complete in this order. Retain the older result as a fallback before merging.
Security architecture risk: Low · up to f86e8
The change restricts stale results without adding permissions or external access. However, an overlapping call that later becomes unavailable can cause the only usable answer to be discarded, leaving the check without an applied result until another evaluation occurs.
Retained concerns
Medium · reliability · inferred: If call A completes while newer call B remains pending, A is discarded. If B subsequently becomes unavailable, it writes nothing and only clears the evidence key; A is neither retained nor replayed. With no prior accepted answer, the question can therefore have no decision or finding, and no associated corrective guidance, until another evaluation occurs. This is introduced relative to the base, which applied A. Clearing the key permits recovery but does not itself schedule it. The unavailable regression covers B finishing before A, not this inverse order.
Security review details
Security Blast Radius
inferred — The demonstrated failure scope is an overlapping trace question within its originating run. Ordering state is owned by that LiveRun and check ID; persistence and steering retain the originating trace ID. The inspected change supplies no new cross-run access or authority path.
Trust Boundaries and Controls
observed — Recheck replaces the live-run object. The new identity predicate rejects results belonging to the previous object, while existing dismissed-finding checks remain in place before result application and after second-look verification.
Resilience and Maintainability Implications
inferred — The unavailable-successor concern is reachable through asynchronous backend failure, not just an instantaneous no-model check: a decision request can remain pending before reporting unavailability. Its demonstrated consequence is loss of a check outcome and potentially its guidance, rather than a verified privilege escalation or tenant-isolation bypass.

Re-run checks replaces the run's live state, so a call still out from
before it kept reading the old evidence key and could write after the
recheck's own answer. Treat a call as stale once its live run is no
longer the current one.

The ordering tests held the older call with a one-second sleep, so a
slow runner could let it finish first and fail the test for timing
reasons. Hold it on a gate the test releases once the newer call has
run.

[P2] Preserve the newest successful answer across an unavailable third call
Clearing the evidence key for an unavailable call makes ?? key authorize every older in-flight call. With three overlapping passes, I held A (yes), let B (no) finish, then let C report DecisionUnavailable and clear the key. Releasing A afterward still inserts its obsolete yes result: persisted probabilities are [0.05, 0.95] instead of [0.05]. Thus the stale finding can still overwrite a newer successful answer. Track attempt/success ownership independently of the transient retry key; allow an older answer only when no newer successful result superseded it. Add this three-call regression alongside the two-call cases.

Good catch, thanks. Fixed in f86e8f8: each trace question's calls are now numbered, and a call writes only when no newer call has written or is still out, so a call that finds no model leaves B's answer standing instead of reopening A. Your three-call case is in the tests (it fails on e08cfc4 with [0.05, 0.95]), plus one for an older answer that finishes while a newer call is still out, which now writes nothing rather than briefly raising a finding.
…ence key
A call that found no model clears the run's evidence key so the
question is asked again, and with three overlapping calls that let the
oldest write over a newer answer: A held, B answered no, C found no
model and cleared the key, then A's yes was stored.
Number each trace question's calls. A call writes only when no newer
call has written or is still out, so a call that found no model leaves
the last answer standing without reopening older ones.

Major · Functional Correctness · Quick win
Retain an older answer until the newer call succeeds.
If call A finishes while call B remains in asks.out, current() drops A permanently. If B then throws DecisionUnavailableError, B clears its evidence key, but no call applies A’s answer. A run with no further spans or end pass can therefore lose its only available decision and finding. Keep A’s completed result as a fallback until B succeeds or becomes unavailable. Add a regression test that releases A before B reports no model.
