chore(models): sync Command Code catalog

[github-actions]

Synchronize the Command Code catalog with command-code@1.56.0.

• Update the CLI version header and documented snapshot.
• Add Qwen/Qwen3.8-Omni-Flash image input, reasoning efforts (low, medium, xhigh), and a 131,072-token output limit.
• Add manually reviewed display prices from https://commandcode.ai/docs/resources/pricing-limits: $0.15 input, $0.47 output, $0.016 cache read, $0 cache write per million tokens.
• Refresh the live model-ID and pricing fixtures, add explicit capability/pricing assertions, and document the change in Unreleased.
• Incorporate current main, including fix(tools): collapse nullable type unions for google/gemini-* models #103 and docs(docs): improve plugin update instructions for OMP #104.

Validation: full npm test (including real Pi against mock API), format:check, git diff --check, and latest upstream catalog comparison pass locally. Local OMP test skips because omp is unavailable; GitHub CI runs the OMP compatibility suite.
