Summary

  • Problem: a gloss's mentions (the MENTIONS edges annotate writes) were tracked by qualified name alone, while its subject is repaired by content hash and text quote. After every sync, CYPHER_REANCHOR_GLOSS_MENTIONS merged an edge to whatever definition carried a recorded name. So:
    • renaming a mentioned definition (cgr rename, or by hand) or moving it unchanged to another file dropped its edge, while the note stayed EXACT and nothing said a mention was gone;
    • an unrelated definition that later took the old name inherited the mention, and glosses reported the note as being about code it never referred to.
  • Fix:
    • At write time: a note records each mention's content hash and text quote beside its name, in mention_qns order (mention_hashes, mention_quotes), as _target_facts already did for the subject. An empty string stands for "none".

    • After every sync: repair_mentions (in gloss_repair) replaces the name-only merge and places every attached note's mentions:

      • name still holds the recorded code: kept by name (the usual case, one read);
      • name gone or holding other code: the recorded code is looked for in the note's project by hash, then by quote. Exactly one home is followed, and the mention is renamed to it;
      • no single home, name still there: the definition was edited in place. It keeps the mention, and its anchors are renewed so a later move is followed from the edited code;
      • no single home, name gone: the mention is listed in a new mentions_lost, with no edge, until its code comes back. Two identical copies count as no single home and are not guessed between.

      A name reused by unrelated code therefore never takes a mention by name alone: not while the recorded code is elsewhere, and not once the mention is lost.

    • Writes: a note is written only when its mentions or edges change. A followed mention is bound only while its new name still carries the hash the pass read.

    • Notes from before this change: they carry no mention anchors. Their first pass records the anchors of the code under each name, so later moves are followed.

  • A Memgraph quirk found along the way: with the definition indexes cgr start creates, a statement that deleted a note's MENTIONS edges and then merged the same edge back lost it.
    • Statement shape: the mention statement matches its targets before any delete and deletes only edges to nodes that are no longer targets, the same shape CYPHER_GLOSS_WRITE uses.
    • Tests: the integration tests create those indexes, which reproduces the lost edge on the first version of the statement.
  • Read side: glosses rows carry mentions_lost. The glosses tool description, the MCP guide and the graph schema (types_defs and graph-schema.md) describe the new properties and behaviour. The eval double lists the new read as not modelled, since it holds no Gloss nodes.
  • Not in this PR:
    • grading a note STALE when a mention is lost (the issue's optional item). mentions_lost makes it visible instead;
    • having cgr rename rewrite mention_qns directly in the same transaction (the issue's item 3). The rename's own re-ingest now follows the mention through the quote tier, as the GIF shows.

Demo

A gloss mentioning b.run through a cgr rename, a newcomer and a delete: main vs this branch

The issue's gm repo is indexed against Memgraph:

  • a.helper;
  • b.run, which calls it;
  • a note on a.helper that mentions b.run, written through the MCP annotate tool.

Then:

  1. cgr -q rename P.b.run run_job;
  2. an unrelated run() is added and synced;
  3. run_job is deleted and synced.

glosses is read through the MCP stdio client after each step.

  • main:
    • after the rename: mentions: [] with the note still EXACT, and glosses b.run_job lists nothing;
    • after the newcomer: the note mentions P.b.run again, and glosses b.run lists the note against the unrelated function.
  • This branch:
    • after the rename: mentions: ["P.b.run_job"], and glosses b.run_job lists the note;
    • after the newcomer: the note still mentions run_job, and glosses b.run is empty;
    • after the delete: mentions: [], mentions_lost: ["P.b.run_job"].

The new tests pass (19).

Test Plan

  • All existing unit tests pass (make test): related suites run, full suite see below
  • New tests added for new functionality
  • Integration tests pass (if applicable)
  • Manual testing performed

Red/green TDD:

  • codebase_rag/tests/integration/test_gloss_mentions_e2e.py: a real Memgraph through testcontainers, with the cgr start indexes. The repo is indexed with GraphUpdater, edited with reingest, and notes are written and read with gloss.write_gloss / glosses_for.
  • codebase_rag/tests/test_gloss_mention_repair.py: the pass's writes against a store that answers only its queries.
  • codebase_rag/tests/test_gloss_tools.py: the write.

Red on main (5f76ace): all 9 first-written integration tests and both write tests failed.

  • A renamed mention: mentions: [].
  • A mention moved to c.py: [].
  • Rename plus a newcomer in one edit: ['gm.b.run'], the newcomer.
  • A deleted mention: no mentions_lost.
  • A deleted mention, then a newcomer: the newcomer took it.
  • The negatives (edited in place, restored, untouched, legacy) failed on the missing mentions_lost field.
  • No mention_hashes or mention_quotes were recorded.

Green on this branch:

  • 13 integration tests and 6 unit tests in the new files;
  • 87 with test_gloss_tools.py (2 new tests there; its reanchor-order test now expects CYPHER_GLOSS_SET_MENTIONS).

Negatives:

  • A mention edited in place keeps its edge, and a later rename of it is still followed.
  • A deleted mention whose code comes back is restored.
  • A re-parse that changes nothing keeps the edge.
  • A pre-change note still attaches by name, gains anchors, and has a later move followed.
  • Two identical copies of a deleted mention are not guessed between.
  • An unchanged mention writes nothing.

Mutation checks: 24 run, all caught. They cover:

  • each placement branch;
  • the hash and quote tiers;
  • the uniqueness check;
  • the renewed hash and quote;
  • the lost flag;
  • anchor alignment;
  • the no-change skip;
  • the bind-by-key for moves;
  • the write and read fields;
  • the stale-only delete.

The first run showed the hash-less fast path to be unobservable (renewing gives the same result), and it was removed. The renewed-hash, unique-home and hash-only cases got tests.

Wider runs:

  • Every non-integration test matching gloss, schema, doc, description, readme, eval, emulator, reanchor or tool passes, apart from main's two known C# toolchain guards.
  • Every gloss integration test passes.
  • ruff check, ruff format and ty are clean.
    • Full suite with Memgraph: queued in the local serial runner; the result is added to this description when it lands. CI runs it too.
  • Manual: the demo above.

Checklist

  • My code follows the Conventional Commits format
  • I have run pre-commit hooks locally: ruff, ruff format and ty were run by hand on the changed files

Summary by CodeRabbit

  • New Features
    • Gloss mentions now follow definitions across renames and moves when uniquely matched by code or source text.
    • Unresolved mentions are reported separately, without attaching to a new, unrelated definition that reuses an old name.
    • Mentions remain attached when code is edited in place and can be restored when the original code returns.
    • Mention records are refreshed during synchronization, while changes to a note made during that process are protected.
  • Documentation
    • Clarified how mention tracking, reattachment, and unresolved-mention reporting work.

vitali87 and others added 2 commits October 10, 2026 01:37
A gloss's mentions were recorded by qualified name only, and after every
sync `CYPHER_REANCHOR_GLOSS_MENTIONS` re-attached them by name. Renaming or
moving a mentioned definition therefore dropped its MENTIONS edge while the
note stayed EXACT, and an unrelated definition that later took the old name
inherited the mention.

A note now records each mention's content hash and text quote beside its
name, as it does for its subject. After each sync `repair_mentions` places
every attached note's mentions: by name while the name still holds the
recorded code; otherwise by hash, then by quote, to exactly one home in the
note's project, which renames the mention; otherwise a name still present
was edited in place and keeps the mention with its anchors renewed, and a
name gone leaves the mention in `mentions_lost`, with no edge, until its
code comes back. A name reused by unrelated code never takes a mention by
name alone. A note is rewritten only when its mentions or edges change, and
a moved binding is re-validated by the hash the pass read.

`glosses` rows carry `mentions_lost`; the tool description, the MCP guide
and the graph schema say how mentions are followed.

Fixes #3230

Claude-Session: https://claude.ai/code/session_01EbsCWQcSTGQXv3g5sk418m
With the definition indexes `cgr start` creates, Memgraph 3 lost the edge
when the mention statement deleted a note's MENTIONS edges and then merged
the same edge back in one statement, so after a real sync a placed mention
had no edge. The statement now matches its targets first, deletes only an
edge to a node that is no longer a target, and merges the rest, the shape
the gloss write itself uses. A followed mention is bound by its name and
hash joined in one key. The integration tests create the indexes, which
reproduces the lost edge on the old statement.

Gloss mentions can attach to unrelated code when the recorded code exists in several places. A mention can also vanish from a note without being reported when another mention is renamed onto its name. Both are edge cases, but each breaks the PR's stated promise that mentions are not inherited by newcomers and that losses are reported. Fix them before merging.

Explanation

The description includes all required sections, explains the problem and solution, documents scope and exclusions, provides a demo, links the related issue, and details tests and validation. Some broader checks remain queued or unchecked, but the description is substantially complete.

Full details: Linked Issues check

Explanation

Directly linked issue #3230 is open. It is the only active target. Its coding requirements and the code that meets them:

  1. [#3230] A renamed or moved mention is followed by hash or quote. write_gloss records mention_hashes and mention_quotes beside mention_qns. repair_mentions in gloss_repair.py places each mention by its recorded hash, then by its quote, within the note's project. The integration test test_a_renamed_mention_is_followed (rename) and test_a_mention_moved_unchanged_to_another_file_is_followed (move to c.py) assert mentions moves to the new name. Test test_a_moved_mention_is_followed_by_its_hash_alone covers the hash-only case without readable source.

  2. [#3230] A mention that cannot be placed is reported, not dropped. A mention with no single home and a missing name is listed in mentions_lost with no edge. test_a_mention_that_cannot_be_placed_is_reported asserts this. GlossRow and the glosses description expose the field.

  3. [#3230] An unrelated newcomer does not take a mention by name alone. test_a_newcomer_reusing_the_name_does_not_take_the_mention and test_a_lost_mention_is_not_given_to_a_later_newcomer assert this. Test test_a_mention_whose_code_went_to_two_places_is_not_guessed covers the ambiguous case.

The issue's tests for renames, moves and name reuse are present in test_gloss_mentions_e2e.py, read at the reviewed head. The optional STALE grading and the rename-transaction rewrite in the issue's suggested fix are not implemented. The issue lists STALE grading as optional. The rewrite is a suggested route, and the expected outcome (following the mention after rename) is met by the repair pass. The PR description states both exclusions.

Limits: I read the integration test file at head. The gloss_repair.py read was truncated before repair_mentions, so the placement branches are assessed from the PR summary and test names. The full Memgraph suite result is not verified here, and the PR reports it as queued.

Full details: Out of Scope Changes check

Explanation

Each changed area supports the mention-anchoring fix for [#3230]:

  • constants/graph.py, types_defs.py, docs/architecture/graph-schema.md: new anchor and mentions_lost keys and schema entries.
  • cypher_queries.py: CYPHER_GLOSS_MENTION_ANCHORS, CYPHER_DEFINITIONS_BY_QNS and CYPHER_GLOSS_SET_MENTIONS replace the name-only CYPHER_REANCHOR_GLOSS_MENTIONS. The set statement follows the Memgraph delete-then-merge ordering fix described in the PR.
  • gloss_repair.py, graph_updater.py: repair_mentions runs in place of the name-only merge.
  • gloss.py, tests/test_gloss_tools.py: write path and the mentions_lost read field.
  • tools/tool_descriptions.py, docs/guide/mcp-server.md: documentation of the new read behaviour.
  • tests/test_protobuf_node_property_parity.py: exemption for properties recomputed after sync.
  • evals/cgr_graph.py: in-memory double accepts the new read query.
  • Test files: coverage for the new behaviour.

No unrelated functional change is shown. The PR's reported CI failures in test_sql_ddl_routine_references.py and the Java oracle test are attributed to main and touch no changed file in this PR's listed scope.

`mention_hashes` and `mention_quotes` sit positionally beside
`mention_qns`, which the export already leaves out because the MENTIONS
edges carry it; `mentions_lost` is recomputed after every sync like
`candidate_qns`. The parity test now names all three with that reason.

The mention pass reads every note, then writes each result in its own
statement, so an `annotate` from another server or watcher could land in
between and have its new mentions overwritten by the stale result. The pass
now reads the note's `write_id` and the write applies only while the note
still carries it.

…till matches

When a definition a mention was followed to was replaced between the
pass's read and its write, the hash-qualified match found no target, but
the statement had already saved the new name and cleared `mentions_lost`;
the next pass then read the newcomer as an in-place edit and bound the
mention to it. The note's properties and edges are now changed only when
every followed key still matches a target, so the note keeps its earlier
name, anchors and lost state and the next sync places it afresh.

…e name

When a note's mention `a` was lost and its other mention `b` was then
renamed to `a`, both placed under `a`; the lost one came first in name
order and won, so the followed mention lost its edge and its anchors were
dropped on the write. A live placement now takes the name over a lost one.

Require positive identity evidence before renewing a same-name mention.

When the old hash and quote have no unique match, _MentionPlacer.place renews the mention whenever the original name has any current definition. An unrelated replacement with the same qualified name therefore receives a live mention edge. This violates the documented no-inheritance contract.

Change the repair contract so _renewed runs only with positive same-definition evidence from the sync. Otherwise mark the mention lost. Add a regression test for direct same-name replacement.

  • Unit Tests (every OS/Python, shard 1/2), SonarCloud Analysis, Sonar Zero Issues Gate, All Checks Pass: the same 12 cases in test_sql_ddl_routine_references.py fail. That file isn't on this branch; it came from fix(sql): routines are dead-code roots, and DDL that names one calls it #3251. Main's own CI has failed these 12 since fix(sql): routines are dead-code roots, and DDL that names one calls it #3251 merged (ee90639, through e790ff5 / 0.3.11), and the commit before it (9ae0872, fix(sql): one statement the grammar cannot read costs only that statement #3254) was green. They fail identically on a clean checkout of main.
    • Cause: fix(sql): one statement the grammar cannot read costs only that statement #3254's parse_sql can re-parse a statement-isolated copy of the file in which statements the grammar can't read are blanked. fix(sql): routines are dead-code roots, and DDL that names one calls it #3251's CallProcessor._ingest_sql_ddl_references scans root_node.text, which is that copy. So the CREATE AGGREGATE / EVENT TRIGGER / CAST / OPERATOR / FDW / LANGUAGE clauses it exists to read are already blanks. Each branch passed CI before the other merged.
    • No fix PR exists yet. A proposed patch: scan the file's own bytes. The copy keeps every offset and newline, so the sites are unchanged. With it applied to main, all 136 SQL tests pass, and ruff and ty are clean:
      -        text = root_node.text
      +        text = self._sql_file_text(root_node)
      ...
      +    def _sql_file_text(self, root_node: Node) -> bytes | None:
      +        text = root_node.text
      + if self._call_file is None or text is None:
      +            return text
      +        try:
      +            source = grammar_bytes(
      +                self._call_file.read_bytes(), cs.SupportedLanguage.SQL, self._call_file
      +            )
      +        except OSError:
      +            return text
      +        return source if len(source) == len(text) else text
  • Unit Tests (base install): test_evals_honour_cgrignore.py::test_the_java_oracle_honours_cgrignore failed with the JVM exiting 1. It passed in main's run at e790ff5 and passes locally on this branch. It looks like the race in evals/oracles/java_oracle._ensure_compiled: its unlocked freshness check can accept an Oracle.class that another xdist worker's javac is still writing. Nothing in this PR touches the oracles.

This PR's own checks pass: Lint & Format, Type Check, Integration Tests, CodeQL, Breaking Change Notes, and shard 2/2 on every OS. The cancelled run on this head has already used its one re-run. Once main carries the SQL fix, this branch will merge main in and CI will run again.

Do not renew a mention when its recorded code has multiple homes.

If two definitions carry the recorded hash and the original name now holds unrelated code, _elsewhere returns both hash matches. place then calls _renewed because the original name still has hashes. The renewed placement is live, so _write_mentions attaches the note to the unrelated code under its original name.

The hash tier returns before the quote tier runs. No other guard marks this active mention as lost. The in-place-edit fallback applies only when the recorded code has no destination. Keep the mention unresolved when the hash lookup returns multiple candidates.

Keep a lost mention when a live mention takes its name.

When one note mentions a and b, a's code is deleted, and b's code is renamed to a, _keep_placement keeps the live b placement on a and discards the lost a placement. _write_mentions then writes only the live placement and clears mentions_lost. The original a record, including its hash and quote, is gone. The note is reported under moved but not under lost.

This contradicts the repair_mentions docstring, which says a lost mention stays in mentions_lost "until its code comes back." If the original a code returns, no mention attaches to it, and the mention is dropped without a report.

A local fix is not available. The write path deduplicates entries by qualified name, and mentions_lost stores names only. Keeping both records requires a different identity for the lost entry in the stored shape, which means a change to the write query and to the readers of that shape.

… several homes

When a mention's name held other code and its recorded hash matched two
or more definitions elsewhere, place() fell through to _renewed and kept
the mention on the name, binding it to the unrelated code there. The
recorded code did go somewhere, so this is no edit in place: the mention
now goes to mentions_lost, as it already did when the name was gone.
A name that is itself one of several quote homes (a decorator change
moves the hash, not the quote) still keeps its mention.

Replies to the two outside-diff findings in CodeRabbit's review on 41a694e:

1. gloss_repair.py:621-626, a mention renewed while its code has several homes: fixed in b207535. Confirmed: b.run holds other code and the recorded hash is on c.run and d.run. _elsewhere returns both, place falls through to _renewed, and the note gets an edge to the unrelated b.run. That breaks the docstring's own rule ("not while the recorded code is elsewhere"). _renewed now runs only when the recorded code is in no other place, so this case goes to mentions_lost. That matches what already happened when the name was gone (test_a_mention_whose_code_went_to_two_places_is_not_guessed). One case still renews: a name that is itself one of several quote homes. That happens when a decorator change moves the hash but not the quote. Regression tests: test_a_mention_whose_code_has_two_homes_leaves_the_reused_name (fails before the fix) and test_a_name_among_several_quote_homes_keeps_its_mention.

2. gloss_repair.py:749-751, a lost record dropped when a live mention takes its name: not changing it in this PR. The path, traced: the note mentions x.a and x.b. One sync deletes a, which goes to mentions_lost. A later sync renames b to a. The hash covers the name, so b is followed by its quote to x.a, and both placements land on x.a. _keep_placement keeps the live one and drops a's hash and quote. That is the behaviour #3329 settled on in the Greptile thread (r4307083036). No edge goes to the wrong code: the note is bound to exactly the definition it now names. What is lost is the old a record. So if a's original code comes back, which needs the same name a in another module because x.a is taken, it isn't followed, and the note no longer lists it as lost. Keeping both records needs two entries under one name, and the stored shape can't hold that: mention_qns, the positional hashes and quotes, and mentions_lost are all keyed by name. The fix would change write_gloss, CYPHER_GLOSS_SET_MENTIONS, CYPHER_GLOSS_MENTION_ANCHORS, the GlossRow reader, the schema string, protobuf parity and the docs. That is a lot for a three-sync sequence that never binds a mention to the wrong code, so it fits better as a follow-up issue than in this PR.
