Summary

• Fix COMMANDS.md (skills/wspace/references/COMMANDS.md): the schema section wrongly claimed workspaces[] was removed in v4 and that path/groups/localFiles/manifest/worktreesDirectory/secretsDirectory are rejected with migration errors — the v4 schema keeps workspaces[] + workspacesDirectory, and the loader ignores all unknown keys except vaultDirectory.
• Restore the missing Conservative mutation design-principle bullet (its tail had been fused into Provider-agnostic).
• Fill command-table gaps: missing init, path, workspaces rows; add missing --as-workspace.
• Remove orphaned fragments (the --no-track/origin/HEAD paragraph, broken Path-rule bullet with empty sh block, dangling sentence tails).
• Resolve the duplicate-name contradiction: same names across workspaces are fine (README + code agree); only same-checkout-path collisions conflict.
• README.md: repair the truncated Conservative-mutation bullet and the "Edits are surgical manifests survive." fragment.

Verified against the v4 JSON schema and the CLI source; deno task ci green (172 tests).
