Wire existing IommuMap::walk / resolve_result hole-on-bound-SID path into make red-team as [redteam] attack=smmu-not-mapped result=refused. Mapped IOVA admits; hole → NotMapped. Not WrongStream / Stage2Fault / StreamAbort / SubmitSid / Overlap.
